The second door guards anentrance to the Underdarknear the Mushroom Circle mini-teleports. There are two ways to encounter this door. You can find it while you’re exploring the Hag’s lair, or you can see it when you’re exploring the Underdark. Here is how to go through both doors.

How To Get Past The First Gnarled Door
There are two methods of getting through this Gnarled Door:
Some players have had trouble getting their character to move through the door. To circumvent this, press and hold LMB and point your cursor through the Gnarled Door. Your character will follow the cursor’s direction to walk through to the other side.
How To Get Past The Second Gnarled Door
After defeating or bypassing the four Mask enemies, you’ll find yourself in a large chamber with Noxious fumes blocking the path ahead. If you explore this area, you’ll be able to find the second Gnarled Door hidden to the side. Unlike the first door, however, you cannot interact or walk through this door. There are only two known ways to get past the Second Gnarled Door.
The second Gnarled Door only serves as a passage from the Hag’s lair to the Underdark. If you encounter this door from the Underdark’s side and you’ve already explored the Hag’s lair and done everything there is to do, there is no reason to head back here; no secrets to be found.
